Sacred Heart's Contract Request Form, field by field
The Contract Request Form (CRF) is how Sacred Heart University learns where you will train, and it starts the contract with that site. You submit it after your preceptor is approved, and it travels straight to the Clinical Contract & Compliance Coordinator. How accurately you fill it in, above all the site's contract contact, shapes how fast the affiliation agreement moves.

All six are online Formstack forms linked from the College of Nursing Forms page. Pick the one that matches your program, because the questions differ, most of all for FNP students, whose form asks about the populations a site serves. When the form goes in
Timing depends on your track. Online NP students wait for the email from the MSN Online NP Programs Clinical Coordinator approving the site and preceptor, then submit. CNL, NE, NMEL and RN-BSN students submit once the academic advisor approves. In the hybrid FNP/DNP, the form is the first step for any independent placement, and DNP students send one for the project site before the end of NU 823.
Early submission matters because the form is the trigger for the contract. Deadlines by program live on the week-10 deadline page, the approvals that come before the form are on preceptor approval, and the full sequence is on the placement process page.
Online NP students submit the CRF only after the NP clinical coordinator's approval email arrives. Sending it earlier skips a step Sacred Heart requires.
What the form asks
The public versions of the form ask for:
- The proposed semester and year, and the course number.
- Your faculty adviser.
- The site's name and address.
- The site's contact for contracts, meaning the person who can sign or route an affiliation agreement.
- Whether you are currently employed at the site.
- Whether you work with the preceptor or mentor in any capacity at your job.
- Whether the preceptor or mentor is a Davis & Henley College of Nursing adjunct.
- For FNP, the primary and secondary population at the site: Primary/Preventive Care, Women's Health, Pediatrics or Specialty Care.
- A required confirmation that your preceptor is not a family member or close friend.
The MSN Online and RN-BSN version also shows the six module windows, from Module 1 (August to October) through Module 6 (June to August), so the request lines up with your course dates. The employment questions are explained on clinicals where you work.
Getting the contract contact right
This one field carries more weight than any other. The Clinical Contract & Compliance Coordinator uses it to confirm an existing affiliation agreement or begin a new one, so a wrong name sends the request to someone who cannot act on it. The preceptor is often not the right person; many practices route contracts through an office manager, an administrator, a legal team or a health system's student placement office.
Ask your preceptor who signs student agreements for the practice, then confirm that person's email and title directly. If the site sits inside a larger system, name the system's placement or contracting contact. A correct contact lets the coordinator start work the moment the form lands. It also helps to let that person know a request from Sacred Heart is coming, so it is recognized rather than set aside.
After you press submit
Sending the form is not a finish line. Here is what happens once it leaves your hands:
- Automatic routingThe moment you submit, the form lands with the Clinical Contract & Compliance Coordinator, with no further step from you.
- Agreement checkThe coordinator looks for an affiliation agreement with the site and initiates one if none exists. A new agreement can take months; see affiliation agreements.
- Contact only when something blocks itThe coordinator writes to your SHU email account only if there is a problem solidifying the agreement. Quiet means no problem has been flagged.
- Compliance and clearanceWith the agreement confirmed, your Health Account compliant and your preceptor approved, the Cleared for Clinical email follows.
While you wait, keep your Health Account current and your SHU email open. The guides give students 48 to 72 hours to answer clinical and compliance messages, and a slow reply to the contract coordinator slows the agreement too.
When you need a new form
Each CRF describes one placement. Switch the site or the preceptor and Sacred Heart wants three things: word to Clinical Placement straight away, a replacement CRF, and a pause until the updated clearance letter comes through. The same logic applies to a second site added partway through a course.
Hybrid FNP/DNP students submit a form for each independent placement. DNP students in both programs submit one for the project implementation site, and post-master's DNP students pair it with a practice mentor agreement form, both due before the end of NU 823. New DNP contracts can need 6 to 8 months, per the DNP guides, which is why they ask students to begin the site search well before that course ends.

By the time your form is due, your placement advisor has already gathered everything it asks for: the site's full name and address, a named contract contact with a working email, the population mix for FNP courses, and a preceptor screened for conflicts of interest. You copy the details in and submit, and the Clinical Contract & Compliance Coordinator begins with a clean request. We also flag early whether the site belongs to a partnering health system, so that system's own approval is under way before your form is due.

Where do I find Sacred Heart's Contract Request Form?
All six versions are linked from the Forms page under Student Resources on the College of Nursing website. CNL, NE and NMEL students also find theirs in the Graduate Nursing Clinical Requirements Blackboard organization, and FNP students find the process and forms in the FNP Clinical Requirements Blackboard organization.
Does submitting the CRF mean my placement is approved?
No. For NP students, approval comes before the form, from the academic advisor and the NP clinical coordinator. The form starts the contract side, the affiliation agreement. You may attend clinical only after the agreement is confirmed, your compliance is complete and the Cleared for Clinical email arrives.
What if my site already has an agreement with Sacred Heart?
Submit the form anyway. The Clinical Contract & Compliance Coordinator uses it to confirm that the existing agreement covers your placement, and Sacred Heart already holds agreements with a very large number of sites. The guides give no confirmation time, so keep an eye on your SHU email in case the coordinator needs something.
Who counts as the site's contact for contracts?
The person at the site who handles affiliation agreements with schools. In a small practice it may be the owner or office manager; in a hospital or large system it is often a student placement or contracting office. Ask your preceptor who signs student agreements, and confirm that contact's email before you submit.
Do DNP students use the same form as NP students?
No. DNP students use the FNP/DNP Hybrid, PM/DNP Hybrid & PM/DNP Online CRF, which covers the project implementation site and is due before the end of NU 823. Post-master's DNP students also submit a practice mentor agreement form. Hybrid FNP/DNP students use the same form for independent clinical placements.
